3. Clinical Records & Protected Health Information (PHI)

All clinical services, session notes, intake forms, billing information, and protected health information (PHI) are stored and managed through a separate HIPAA-compliant electronic health record system: TherapyNotes.

TherapyNotes maintains its own privacy practices and Notice of Privacy Practices (NPP). Clients receiving services will be provided with appropriate HIPAA documentation directly through that platform.

Our website does not collect or store protected health information.

If you consent to receive SMS from Me, Myself, & Mind, LLC (M³), you agree to receive appointment reminders, scheduling communications, service-related notifications, and responses to inquiries via SMS from us.
